MiiVi webpage tweaks: legal + analytics

Randy would like a link to MiiVi's Terms of Service (www.miivi.com/terms) be displayed at the bottom of every MiiVi page (as YouTube does). Also, similar to the Queue Generation popup that warns that your Queue is still being generated when people first Generate New Queue, when users change their Filter Level to either "Moderate" or "No Filter" a popup should appear with "I Agree" and "I Decline" buttons below the Filter Level change warning. Attached please find the texts for the Terms of Service (miivi-ToS.doc) and the Filter Level (miivi-Moderate_or_No_Filter.doc).

Lastly, Randy would like every page on MiiVi to be tracked with some sort of web analytics. As we have the most experience internally with Google Analytics, here is the tracking code that should be displayed on each page (I've been told by ArtistDirect people that analytics should be placed at the bottom of web page code to reduce load time on page in the event the analytics' java script call hangs for some reason):
